INDIVIDUAL COMPANY: Registration with the Board of Trade

COMMERCIAL COMPANIES: Constitutive Act, Statute or Social Contract in force, duly registered.

SOCIETY FOR SHARES: Constitutive Act, Statute or Social Contract in force, has been registered, accompanied by documents of selection of its administrators.

CIVIL SOCIETY: Registration of the Constitutive Act in the Civil Registry of legal entities, when it does not exist, in the Registry of Securities and Documents, accompanied by evidence of board of directors in exercise.

COMPANY OR FOREIGN COMPANY OPERATING IN THE COUNTRY: Decree of authorization, and act of registration or authorization for the operation issued by the competent body, when an activity so requires.

Registration or registration with the competent professional entity (eg, UNIONS, ASSOCIATIONS, REGIONAL COUNCILS, CREA, CRF, etc.);

Certificates of technical capacity issued by companies that have provided services or sold products (at least two certificates);

For bidders who register to provide health products (hospital medical supplies, orthotics and prostheses and laboratory supplies) and medicines, they must also present:

Authorization of Operation of Company (AFE) issued by ANVISA.
Proof of licensing of the competent sanitary body (State or Municipal).

Proof of registration with the National Registry of Legal Entities (CNPJ), with validity;

Proof of regularity with the farms: Federal, including Active Debt of the Union, State and Municipal;

Certificates of Fiscal Regularity with the FUND OF SERVICE GUARANTEE – FGTS;

Negative Debit Certificate proving regularity with the INSS.
